• A Purchase/Check Request form that must be completed, signed, and returned to the church office before you purchase any item for the church.

• A Non-Cash Gift form which you can use for gifts or items that you buy and give to the church. If for instance, you host a youth dinner on Sunday night, you can use this form to get credit for that gift on your giving statement.
